1. Data Application

I. For first-time applicants, please refer to the "Codebook" to understand the database content and confirm the criteria for specimen selection to facilitate your application process.

II. Since the research variables and sample selection criteria required for each case differ, you may submit your "Selection Criteria" and request to "retain the necessary variable fields in the Codebook" via email to [email protected] before applying. We will provide you with the relevant "Basic Descriptive Statistics" and a quote based on your research needs. You can adjust your variable requirements and selection criteria based on the descriptive statistics.

III. When submitting the database usage application, if you need to apply for bio-specimens along with data, please ensure that you retain the necessary variable fields in the Codebook and prepare the following documents in electronic format to be emailed to [email protected]:

1. Qualification Documentation of School or Institution (refer to Section 2.1 of the Application Guidelines for eligibility).
2. IRB certification documents.
3. Material Access Application Form.
4. Material Use Project Proposal.

2. Contract

Once the application is approved, the applicant will be notified to pay the management service fee and sign the database usage contract. Please print and complete the following forms, then mail the originals to:
